- Eligible after one year of continuous service
- You may purchase an additional 15% eligible pay of LTD coverage (total 65% when combined with Basic LTD)
- EOI required if you don't enroll when first eligible
- Maximum monthly benefit equal to $25,000
- Employee paid (post tax)
