• Easy Online Health Assessment:  This free online Health Assessment offers you immediate information and helps identify health risks. Taking the HA is simple, convenient and at no cost to you. The HA is completely confidential and your personal results will be provided immediately. You will see a summary of your health habits and recommendations on how to improve or maintain your healthy behaviors. If you have taken the StayWell Health Assessment in the past, you will be able to compare your current results with the past to see how your health has changed over time.

  While you're there, check out other tools and resources available to you on the StayWell wellness site. You can access online health centers on topics such as:  blood pressure, cholesterol, physical activity, tobacco cessation, weight and many others that will provide information to help on your journey to good health. You can also set up goals, have personalized reminders emailed to you as you work on your health habits, take quizzes to assess your knowledge, access articles on health topics and much more.

• 24 hour Nurse Advocate Line:  Call 866.988.WELL (9355). Caring, experienced registered nurses are there for you 24 hours a day to answer questions, offer support and help you with any health care issue or concern. It is 100% confidential, so call anytime that health is on your mind. Administered by SHPS.



• Condition Assistance Programs:  Trained nurses help you manage chronic health conditions - such as diabetes, heart disease, high blood pressure and asthma - by providing support, information and advice. We can communicate with your doctors if needed, plus keep you up-to-date and on-target. Administered by SHPS.



• Preventive Medical Care:  100% coverage for annual screening tests for cancer, heart disease, diabetes, osteoporosis, etc. based on a participant's age. Available in the United Healthcare and Highmark BCBS medical plans.



• Preventive Dental Care:  100% coverage for exams, cleanings and x-rays twice per year. Application of fluoride and sealants for children are also covered at 100%. Available in both Delta Dental plans.



• Preventive Vision Care:  100% coverage after a $20 copay for an annual eye exam. Available in the VSP vision plan.



• Healthy Pregnancy Program:  100% coverage for participation in an education program to ensure a healthy delivery. Available in the United Healthcare and Highmark BCBS medical plans.
