United Healthcare Secure Horizons Plan

Eligibility Requirement:  Legacy CenturyTel retiree over age 65:  1) enrolled in the UHC Secure Horizons Plan in 2009, or 2) enrolled in the United Healthcare Under Age 65 PPO Plan and turning age 65 in 2010.

United Healthcare CDHP Medicare Supplement Plan

Eligibility Requirement:  Legacy CenturyTel retiree over age 65:  1) enrolled in the United Healthcare CDHP Medicare Supplement Plan in 2009, or 2) enrolled in the United Healthcare CDHP and turning age 65 in 2010.

Highmark Medicare Supplemental Plan

Eligibility Requirement:  Legacy CenturyTel retiree over age 65 and you reside in Indiana, Michigan, Nevada, North Carolina, Ohio, Pennsylvania, South Carolina, Tennessee and Virginia and you were not enrolled in the UHC Consumer Driven Health Plan in 2009.

How the CDHP Works N/A N/A When you turn 65, you are no longer eligible for the annual Health Reimbursement Account (HRA) contribution. However, you may use any remaining funds in your HRA to pay your out-of-pocket costs, including prescriptions.

If you have retiree and spouse coverage and your spouse is under age 65 (or vice versa), you will continue to receive the HRA contribution for your spouse until he/she also reaches age 65. Eligible out-of-pocket expenses will be deducted from the HRA for both you and your spouse.
