Most of these benefits are provided to employees, but depending on where you work, your job arrangements and other factors, not all of these benefits may be available to you. In addition, many other valuable benefits offered locally are not included here.

It is important for you also to read the Privacy Notice because it concerns all the CenturyTel health care plans. This document explains how medical information about you can be used and disclosed in addition to how you can access that information.
