Action required: Prudential supplemental life insurance evidence of insurability (03.23.10)
Summary
- Some employees who enrolled in supplemental life insurance during Open Enrollment will be receiving a follow-up e-mail from Prudential on Wednesday, March 24 regarding updated evidence of insurability verification. Employees who elected supplemental life insurance for your spouse will receive information from Prudential at your home address.
Who does this affect?
- Employees who enrolled in supplemental life insurance during Open Enrollment must provide additional information to verify their eligibility for coverage if:
- You had no coverage in 2009 and applied for coverage in 2010
- You had less than 1x pay in 2009 and applied for coverage in excess of $300,000 for 2010
- You had coverage in 2009 under $500,000 and applied for coverage in excess of $500,000 in 2010
Employees who enrolled in spouse supplemental life insurance during Open Enrollment must provide additional information to verify spouse eligibility for coverage if:
- Your spouse coverage amount in 2010 is in excess of $50,000
- Your spouse had no coverage in 2009 and applied for coverage in 2010
- You increased coverage for your spouse by more than one level from 2009 to 2010
What action is needed?
- If you receive an e-mail or letter from Prudential, you must respond within ten days of receiving the e-mail notification
- If you are not contacted by Prudential no action is necessary
