Lifestyle Benefits allow you to enhance your existing CenturyLink benefits with insurance and other services that can provide financial protection against life’s uncertainties. These voluntary benefits provide discounts on a broad range of insurance options and other services you use every day, in addition to convenient new purchasing options which will be explained in more detail prior to the enrollment period.

Why enroll in the Lifestyle Benefits program?

Voluntary benefits offer coverage that is not included with your core benefits plan. This additional set of insurance, discounts and services offers supplemental protection for yourself and your family. The program is flexible, allowing you to select only the benefits you need

Who does this affect?

• All U.S.-based CenturyLink employees scheduled to work a minimum of 30 hours per week (regular full-time employees), excluding legacy Qwest bargaining employees.

How do I enroll?

• Beginning January 17, eligible employees can sign up for Lifestyle Benefits.


• The special enrollment period for supplemental benefits is January 17 through February 10, 2012.

Where can I get more details?

• Look for information about your Lifestyle Benefits program (voluntary insurance options) in January. The Smart Savings Discount Mall and Purchasing Power programs are available 24 hours a day, seven days a week throughout the year.


• Once you receive the information, look for details on the following:
  --Special group rates for your new lifestyle benefits
  --Simple and affordable payment options, including payroll deduction

• Enrollment counselors will be available beginning January 17. Once enrollment begins you can contact an enrollment counselor at (855) 720-9930 from 8 a.m. to 5 p.m. Central Monday through Friday.
