Summary
If you are enrolled in a CenturyLink UnitedHealthcare or Highmark Blue Cross Blue Shield medical plan you will be eligible for up to $200 in incentives. If you have a spouse or domestic partner who is employed by CenturyLink and covered by one of these plans, they also are eligible to participate. You must complete the voluntary online health assessment and/or biometric screening between January 1, 2012 and March 31, 2012 and be actively employed at the time of the payment in May.

Who does this affect?

Regular full-time employees covered by a United Healthcare or Highmark medical plan. This program does not apply to legacy Qwest bargaining employees.

What action is needed?

Eligible employees who complete the Health Assessment by 10:59 p.m. Central on March 31, 2012 will receive a wellness incentive of $100 in their May paycheck.

In addition, employees who complete the Biometric screening will received an additional $100 incentive in May. The last day to order a home kit or physician form is March 2. The kit and form must be received by your wellness provider no later than March 12. This means the information must be mailed prior to March 12 in order for the materials to be received by United Healthcare or Highmark on time.

Spouses/domestic partners of eligible employees who are not employed by CenturyLink may access and complete the online Health Assessment, but are not eligible for the incentive. Biometric screening is only available to employees.

Health Assessment Instructions:

If you are enrolled in a UnitedHealthcare administered medical plan:

• Log on to www.myuhc.com.


• If this is your first time logging on, you'll need to register on the site.


• After you've registered and logged on, click on the "Health Assessment" link.


• Complete the Health Assessment. You'll receive your health risk score and report card right away.

If you are enrolled in a Highmark Blue Cross Blue Shield medical plan:

• Log on to www.highmarkbcbs.com.


• If this is your first time logging on, you'll need to register on the site.


• After you've registered and logged in, click on the "Your Health" link.


• Scroll down to "Wellness Profile" and click.

Biometric Screening Instructions:

If you are enrolled in either a UnitedHealthcare or a Highmark Blue Cross Blue Shield medical plan:

• Log on to https://individual.wellness-inc.com/centurylink.


• Select one of three options to obtain your biometric screening: Register for an onsite event, if available at your work location; request a kit to be mailed to you; or print a form to take to your doctor and fax it back.


• You will receive your results for key health markers based on your height, weight and blood tests.
  Note: If you participate in an onsite event, you will receive your results immediately at the event.


• If you request a home kit, your results will be mailed to you approximately two weeks following your return of the kit (postage paid).


• If you go to your doctor's office, your results are usually available within a few days. Simply ask your doctor to complete and sign the pre-populated form and fax it back to the number listed on the form.
