Disability Management

CenturyLink provides short and long-term disability benefits to employees who have special circumstances that require time off from work. These benefits include continued pay, medical and other benefits for those who meet eligibility requirements. More information regarding short-term disability (STD) and long-term disability (LTD) benefits for represented and non-represented employees is provided below.

Important Note: The Legacy CenturyLink intermittent FMLA, Short-term disability (STD) and Administrative Leave forms are no longer available on-line. Contact CDS (CenturyLink Disability Services) at 800.729.7526, option 1 then option 4, for the following:
