Affordable Care Act (ACA)
The Affordable Care Act (ACA) requires that employers, like CenturyLink, offer minimum essential healthcare coverage to their full-time employees and dependents.
To demonstrate compliance with these ACA requirements, the IRS requires CenturyLink to provide employees with Tax Form 1095-C. This form is an annual tax statement provided to individuals who were offered or enrolled in health insurance through their employer. The resources on this page will help you to better understand Form 1095-C; what it is, why you are receiving it and what you will need to do with it when it comes time to file your annual tax return.

All CenturyLink employees eligible for insurance coverage in 2016 should receive Form 1095-C. While CenturyLink is only required to offer healthcare coverage to full-time employees (working at least 130 hours in any calendar month) under ACA, CenturyLink offers medical coverage to all employees including part-time and temporary.
Eligible employees who decline to participate in the CenturyLink medical plans will still receive a Form 1095-C.
Helpful ACA Information:
The default is to receive a paper Form 1095-C, however, employees can elect to receive their Form 1095-C electronically (similar to a Form W-2). If you have chosen to receive your 2016 Form 1095-C electronically, it is available to view/print now.
